Even the best players in a broken system will fail. If we do not address the system issues, people performance will continue as is. The challenge is helping leadership understand that current processes, even though successful in the past, may today be driving error in to the system.
Root cause analysis (RCA) is a well-developed, systematic process to find initiating actions or "triggering events", that if not corrected, can have catastrophic results on an organization.
The first type of RCA is typically initiated after a significant event occur. We call these "self revealing" events because "they smack us right in the face." Self revealing events can range from fatalities to large financial crisis or regulatory actions.
The second type of RCA is more proactive, but requires an organization to invest in low level trending of seemingly insignificant events that when analyzed in total, tell you that if no action is taken, bad things are going to happen.
The important components of a good RCA is developing precise problem statement, conducting a fact-based investigation, applying proper analysis tools, determining the root cause, and then developing corrective actions to prevent recurrence.
We are experts in establishing and assessing technical training programs - especially those requiring accreditation or external reviews. We have designed new programs as well as recover broken training programs, including training staff. Using industry proven models, such as Training System Design (TSD) and Systematic Approach to Training (SAT), we analyze, design, develop, implement, and evaluate training program effectiveness.
- Following a thorough assessment, gaining management commitment, and implementing solutions, one organization saw their OSHA injury rate fall from 2.0 to 0.1 - a 20 fold improvement in performance and more workers going home injury free.
- A large generating plant was experiencing five outages per year, most due to operator and technician errors, costing on average $7.2M per outage in lost output. After training management and staff, as well as giving field workers the right to stop work when uncertain, outages declined over time to one every five years.
- One engineering organization experienced a 75% reduction in drawing errors, which lowered issues during field installations saving time and rework expenses.
- A significant event at a nuclear power plant resulted in a regulatory hold on operations until an investigation and effective corrective actions were in place. We conducted a formal root cause investigation and found that the particular process used for years was at fault and workers had just "been lucky" in the past. One simple and inexpensive corrective action provided additional operational margin, lowered risk, satisfied the regulator, and was effective in future operations.
- An electric utility analyzed their "risks to operation" and "risks to financial success." Using a team approach, they identified the components and potential environmental changes that would have the most impact financially. This provided focus to the areas they needed formal action plans and budgeted monies and thereby lower (manage) their business risks.
