Even the best players in a broken system will fail. If we do not address the system issues, people performance will continue as is. The challenge is helping leadership understand that current processes, even though successful in the past, may today be driving error in to the system.
Root cause analysis (RCA) is a well-developed, systematic process to find initiating actions or "triggering events", that if not corrected, can have catastrophic results on an organization.
The first type of RCA is typically initiated after a significant event occur. We call these "self revealing" events because "they smack us right in the face." Self revealing events can range from fatalities to large financial crisis or regulatory actions.
The second type of RCA is more proactive, but requires an organization to invest in low level trending of seemingly insignificant events that when analyzed in total, tell you that if no action is taken, bad things are going to happen.
The important components of a good RCA is developing precise problem statement, conducting a fact-based investigation, applying proper analysis tools, determining the root cause, and then developing corrective actions to prevent recurrence.
We are experts in establishing and assessing technical training programs - especially those requiring accreditation or external reviews. We have designed new programs as well as recover broken training programs, including training staff. Using industry proven models, such as Training System Design (TSD) and Systematic Approach to Training (SAT), we analyze, design, develop, implement, and evaluate training program effectiveness.
